> Who We Are
>
> The New York City Pediatric Disaster Coalition (NYC PDC) was founded in 2008 under the NYC Department of Health and Mental Hygiene (NYC DOHMH). The PDC was established to build a coalition of hospitals, public health, municipal services and community groups to ensure effective use of critical assets during and after a large scale disaster affecting children. The PDC is comprised of experts in emergency preparedness, pediatric critical care, surgery, emergency medicine, representatives from NYC DOHMH, NYC Office of Emergency Management (OEM) and Fire Department of New York (FDNY). Currently 18 of 25 major NYC Pediatric Critical Care (PCC) capable hospitals are participating in the coalition.

> The PDC is headed up by Co-PI George Foltin, Vice Chair, Clinical Services at the Maimonides Infant and Children’s Hospital of Brooklyn, PI Michael Frogel, MD, Program Director Emily Raisch and Project Coordinator Nora Caplan.

> The PDC’s primary goals are to build a coalition for New York City of hospitals, public health, municipal services and community groups to ensure effective use of critical assets during and after a large-scale disaster affecting children

> What We Have Accomplished
>
> Pediatric Surge Planning and Drills
> There are 25 hospitals with PCC services in NYC, with a total of 233 Pediatric Intensive Care Unit (PICU) beds. From the years 2009-2012 eleven major hospitals with a total of 141 PCC beds have used our surge plan guidelines in creating their PCC surge plans, thereby adding 159 surge beds (67% bed increase once the plans are operationalized). Three additional hospitals are in various stages of plan development. Five of those hospitals successfully tested those plans through table top and full scale exercises.

> Prehospital Triage Criteria
> The PDC has formulated and implemented a Pediatric Triage/Transport Plan in collaboration with FDNY/EMS. It includes:
>
> A modified START MCE triage algorithm that adds 5 rescue breaths for unresponsive children and an up-triage option for those in respiratory distress.
> Specific pediatric MCE triage criteria that match acuity and severity of illness to appropriately tiered pediatric disaster receiving hospitals (PDRHs)
> Evacuation and primary transport to these hospitals
> A pediatric intensivist consultant for secondary transport.
> These specific pediatric triage and transport protocols have been integrated into regional EMS system protocols. Approximately 3,000 EMS personnel and 12 pediatric transfer consultants have been trained in these protocols.
> Hospital Destination Criteria
> A list of Pediatric Disaster Receiving Hospitals was created and divided into Tier 1 and Tier 2 based on resources. Tier 1 and Tier 2 pediatric disaster receiving hospitals are staffed by specialized pediatric emergency medicine physicians and pediatric surgeons, specialized pediatric nurses and therapists, emergency department medical staff and nurses who work as a coordinated team to manage treatment and facilitate the recovery of pediatric patients.

> Interhospital Transport Criteria
> The PDC has established a Pediatric Intensivist Response Team (PIRT) in partnership with FDNY for the purpose of recommending secondary transport priority to FDNY. A priority rating scale was created for this purpose, which will allow PIRT physicians to remotely assess patients and rate their need for inter-hospital transportation during an emergency.

> Resource Directories
> In coordination with the NYC DOHMH we have created a New York City Hospital Pediatric Resource Directory and a New York City Neonatal Intensive Care Unit Resource Directory. The directories list all the known hospitals with pediatric in-patient pediatric resources including PICU, General Pediatric Medical Surgical Beds, NICUs, pediatric emergency departments, and well-baby nursery’s including hospitals that will receive transfers to those units. > Neonatal Intensive Care Unit Surge and Evacuation Planning
> The PDC has developed template NICU surge and evacuation plans, along guidance and recommendation documents reflecting lessons learned from Hurricane Sandy.
>
> Hurricane Sandy Response
> Hurricane Sandy hit many members of the PDC whether it be at their homes or their places of work. We have created a working group which includes individuals from hospital administration, physicians, nurses, government organizations and community groups that responded to Hurricane Sandy. This group offered pediatric specific insights which were compiled into lessons learned and best practices documents for the NYC DOHMH to be used for future disasters.

> The PDC is working with hospitals across NYC to develop and implement pediatric critical care surge plans. Two hospitals will also, with PDC support, conduct full scale exercises to test and operationalize their PCC Surge Plans.
>
> Neonatal Intensive Care Unit Surge and Evacuation Planning
> The PDC is working with NYC area hospitals to develop NICU Surge and Evacuation plans. The PDC will assist one hospital with conducting a full scale exercise to operationalize their NICU Evacuation Plan.
>
> Labor and Delivery Emergency Planning
> This year, the PDC is assembling a Labor and Delivery committee in order to compile and create guidance and recommendations for NYC hospital’s Labor and Delivery Units for disasters. The committee will also create template Labor and Delivery disaster plans for use at individual hospitals.
>
> Coalition Building and Community Preparedness
> The PDC prioritizes partnerships with community groups and coalitions. Knowledge sharing, coordination, and joint projects are planned for the year to come.
>
> Special Projects
>
> Participated in Citywide H1N1 Response
> The PDC was asked to support the NYC DOHMH’s response to H1N1. A 311 nursing triage hotline was created along with a CDC Emergency Department H1N1 triage prioritization protocol. The PDC also provided ongoing subject matter expertise on pediatric H1N1 issues.
>
> Haiti Response
> The PDC was involved in information sharing telephone conferences and also liaised with groups on the ground in Haiti for supplies and personnel.
